They go to 3rd world countries (Latin America) and have the surgeries performed by medical school drop-outs. Which also explains why the work looks so terrible.

Also a lot of it falls under body modification, not surgery. So the laws surrounding it are different.

I think in the UK you can have almost anything done "body mod" wise. It has to be without anaesthetic though. I think it's a loose loophole they operate under.

Sure, though they definitely use anesthesia for nose and ear removal.
