Insurance Companies That Cover hCG and Cialis

I became self-employed a little over a year ago and COBRA'd my health insurance. The COBRA will expire in a few months, so I will be paying for my own health insurance at that point. Since the world is my oyster to some degree, I'd like to find a provider that will cover test cyp as well as hCG and/or Cialis for daily use.

Does anyone currently have any providers that will cover these medications?
