Well, let's see here:

I think it's absolutely fine as long as both parties in the "relationship" agree with it. It's an adult decision, made by 2 adults. Getting to explore your fantasies and sexuality is a benefit of being in a trusting and loving relationship. If one or both or you are not interested or can't handle it, then it's a mistake. Otherwise, go for it.

I'm afraid I have to disagree as well. There are many reasons 3-somes occur...and while insecurity certainly might be one of them....it is by means not the only one. Nor would I agree that it is completly detrimental to a relationship providing everyone is in agreement and the relationship is solid.
