Staying on gear for a year....

I been reading posts and saw that some of the well known bodybuilders when they were young would stay on gear for a straight year(no break) and also they talked about some just reg. guys who did this and they became monsters after beinf on for a yr. straight.
My question: How can these people stay on for a year and still make gains? I mean isn't the reason for PCT is that you really not making gains anymore so you get your bod back to norm. and then start over again.
Can someone help explain this to me?

If you keep switching up on the compounds and raise the dosages to the roof you will still make gains after a year. I'm sure that can't be good in the long run.

In addition, it would require a great deal of knowledge of your own body and constant bloodwork to make sure you aren't killing yourself.
