Which Hormone test to ask for

I was at a hospital lab today. I had doctor's orders for a slew of tests and the bill was quite high. I asked what it would cost if I only wanted a hormone profile. The head nurse said that there is no "typical hormone profile". She said you must specify each test you want to have done and it needs to be ordered by a physician (doctor). So, I was wondering what profiles do people ask for when trying to find the problem for their broken libido?? I assume .....

Testosterone Total and Free
Estradiol (estrogen)
Progesterone
Prolactin

anything else ??
