Has anyone had any problems with small crystals forming afterwards?

Would boiling it again fix the problem? It started off milky looking, once added the alcohol is started to turn clear. After a bit it's forming little crystals, I've never done it like this before, I'm assuming it's crashing. Has anyone had this problem? And what's the solution?
