how do infections occurr when injecting?
how do infections occur when injecting? is there bacteria on the area where people administer injections, or do people accidentally touch the tip of the needle before injecting?
i mean if people follow the directions of applying alcohol in the area, and injecting with a sterile needle, how are infections occurring?
or is it from dirty gear?

It can happen from both situations you spoke of.
Most will occur from bad gear.

Infections are rare when you look at it statistically. Infections usually can come from bad gear and in some cases come from unsanitary injection practices (reusing needles/syringes, dropping needle, touching the needle, injecting while in an unclean enviornment)

when numerous kinds of bacteria enter the body you can get an infection. you have bacteria on your body almost constantly and its harmless and unless it enters the body thru a cut or injection. thats why you supposed to swab before injecting. once you puncture the skin the bacteria can enter. obviously shooting bacteria infected gear into your body can cause it as well
