Bloods question about finding a place that will do my bloods

I Live on Oahu hawaii, and i can't do my bloods through my military plan, to dangerous. i need to see a private doc, or something. my GF says that the clinic she goes to can perform bloods.

how do i request this, from one of these docs?

can i tell him/her i'm planning to do over the counter AAS? [i'm not giving my source to him, just because i don't want anyone poking their nose around.]

do i just tell him what i want exactly because obviously this person will be dumb about it.

can i have a list of things to check?

i want to do bloods on cycle as well.
