    Should I See A Different Doctor?

    Long story short:
    40 years old
    200lbs
    4 years in gym, 2 on cycle
    Typically run 500mg sust and 50mg dbol w/ 125mg Sust for PCT/TRT

    Last August I started my first tren cycle: 400mg w/ 750 sust
    10 weeks in started developing very bad acne on my shoulders and back. I've had a little here and there before, but this was bad. Also having very bad night sweats. But, my libido was crazy high and the gf loved it! 3-4 times a day, every day.
    Cut out tren immediately due to acne and lowered test to 500.
    Acne kept getting worse! Sea salt baths and acne creams did nothing. My zits had zits. Also had many cysts.
    Cut test to 250 and went on .25 adex for about a month.
    December cut test to 125
    No improvement to acne. Only got worse. Hurts to sit/lay down, anything. Can only wear black shirts due to leaking and bleeding. It's fucking disgusting.
    January 4th - started Accutane at 20mg. Jan 20 upped it to 40. Feb 5 upped it to 80 (not going any higher).
    Acne is getting better, slowly but surely. Still can only wear black and it hurts like a sumbitch. Another problem is I now have NO LIBIDO WHATSOEVER! Cialis helps me function when the lady can't do without, but really, I could go without it forever, it feels like.

    Went to the doc and explained my situation. He put me on 30 days of Bactrim. I asked for a lab to check hormones. Just got the call today that everything is normal. Doesn't sound normal to me.
    Testosterone at 256 and Estrodial at 43

    Seems like the low side of test and the very high side of estrogen. Anybody got any opinions/suggestions?

    YES,,,see a different doctor. Remember they work for YOU!!! I would say at 256ng/dl and 40 yrs of age that your a perfect candidate for TRT

    From personal experience accutane takes up to 2 months to kick in (And actually makes things drastically worse those first 2 weeks)

    On any AI's? that much sust I'm surprised you don't have a pair of pamela anderson titties to go with that cystic acne

    LMAO, evanescent, no, no AI's and no baywatch titties. Kept Letro and Adex on hand but never needed them.
    And yeah, it definitely got worse in the first month of Accutane, but I can definitely feel it starting to dry out. It's dry and itchy as hell! You just gotta be super careful about where you're scratching.

    Thanks for the heads up on the TRT. I went in for tests about 8 years ago for no libido, and it was in about the same range. He wouldn't recommend TRT and tbh I didn't even know to ask. Lack of sex drive is what killed my last marriage and I'm not going to let it happen again!

    I was thinking about going back on 125mg sust just to raise my test a bit to see if that would help, I'm just scared to do anything with this acne.

    I just feel like that estrogen level is too damn high as well.

    I haven't heard many good things about sust to be honest, and apparently it aromatizes like fuck (and you weren't running AI's)

    You didn't run PCT of Nolva/Clomid either?

    Try get yourself some carmex lip balm (it comes in a yellow tube and tastes like ass). Literally the only thing that stopped my lips from cracking apart and bleeding while on accutane. And avoid alcohol like the plague if you wish to retain your liver. It's surprisingly hard to get accutane prescribed, carries a black box label suicide warning because it can cause depression though that reputation was caused by a handful of nutbags who were on it.

    Consider running a pct. Either standard or a Power PCT Protocol. Use the search function and you'll find it here.
    One more thing, always run an AI on cycle. It's absolutely ridiculous (and honestly stupid) to not do so, unless you like having the estrogen level of a menstruating female.

    A few years ago now. When i started getting fluid coming out of my nips and could not "finish" it hit me like a freight train. estro and prolactin really do matter

    Accutane at high dosage, as you are taking, can crush libido. If thats the case no amount of test will help.

    Nowadays dermatologists are using accutane at much lower dosages as the high dosages come with really bad side effects.

    How many times have you taken bloodwork after accutane started. It is standard once per month? How were your AST and ALT on such dosages?

    Like said above it will take a couple of months for the acne to start improving, in fact it gets worse before it gets better.

    You should not take anything but a TRT dosage with such harsh treatment. Accutane (isotretinoin) is not something to play with, it is a sort of chemotherapy, but many times the only solution for acne.

    Final note, your wife needs to be sympathetic and supportive, and not wanting it all the time when you are feeling down. Thats not love, buy a dildo ffs.

    Sorry, to have to correct you but estrogen peaks at ovulation, during menstruation is at its lowest.

    See a different doctor!

    This is a perfect example.
    Just recently a friend of mine finally took my advice and went to the same endo I use, who's a specialist at male reproductive system.
    He did a bloodtest there, and it revealed the same values that he had with his GP doc. T values and E values were both low, but T values were within the "normal" range on the lab test, but lower normal. LH and FSH were also quite low. He got prescribed T immediately by the endo.
    Had he simply continued going to the GP doc he would never have gotten TRT,
    and his relationship would have turned worse and worse, as he had lost almost all interest in sex.

    When using accutane that can come with its own side effects,
    and this you can explain to your wife/GF/partner/pet sheep or whatever,
    but letting your libido remain low by not seeking TRT, that's on you.

    If I had a GF who had permanently low libido, f.ex by using some birth control pills, id expect her to switch type of birth control.
    If not, she wouldn't have anything to complain about when i fucked someone else. If she had low libido cause she underwent some type of treatment,
    (Like your accutane treatment), then that's a temporary thing and while difficult she could expect me to wait.

    if one part in a relationship always get turned down, that leads to a host of problems. If it's a temporary thing and you talk about it, then ok.
    But in the long run it's a recipy for disaster.

    Drink plenty of water to flush those toxins out, end your showers with cold water to.close those pores, absolutely take yourself to a different doctor explaining what you need instead of telling him what's wrong...

    its got alot more sides, on a genetic level. i would never use nor rec accu to anyone after all the research I have done. you are better off trying a strong antibiotic, couple runs if need be. but OP, yes try a new doc. and also i agree estro could be an/the issue possibly.

    I know i had it bad once and upping my AI and using an organic natty soap ONCE a day helped, like night and day. DO NOT over wash, your body/skin is not stupid it will push more oil out and if already blocked, acne will get worse. 1-2 showers/washes a day MAX IMO. shower in morning and salt bath before bed sounds good

The exogenous test most likely told your body you're a teenager again. Well, a SUPER teenager. Dermatologists put patients on an accutane treatment for a number of weeks to "reset" their glands.

I didn't read anything about PCT so I'm assuming that by lowering your exogenous Test in your body, which has stopped producing natural Test, has significantly lowered your Total Test and Free Test levels. Add to that your Adex and your body doesn't amortize the Test. I'm actually a little surprised your E2 level is where it is. I would have expected your E2 to be quite low. You must've had a ton of E2 already.

    Normal range for Test is: 300 ng/dL and 1050 ng/dL. Normal range for Estrogen is: 20–55 pg/mL. So you're low Test and High E2. No libido, those numbers will do it to you. I'm assuming you're seeing a GP? A GP's good if you have the flu or need a wart taken off. They're not endo's and most know very little about HPTA. You need to go see a specialist. There are other readings that need to be taken into account. I'm not confident that a GP knows enough to make an accurate diagnosis. The first I'd do is stop taking Test, wait 2 weeks, run a full 4 week PCT with Nolvadex and Clomid @ 40/20/20/20 and 100/50/50/50 (mg's each day). At the end of PCT, see how you feel. You main goal is to stabilize your numbers right now. When that happens, your acne should be under control. Good luck.
