100 ml/week is a very low dose, you won't see many sides, like gyno or hair lost. Arimidex will do the work to keep estrogen levels down. At your age, I believe you need higher dose, like 200mg/week or more, you can start at 100mg/week and rise up.

I'm 52, had gyno at puberty, started HRT with 100 ml of Test Cyp 50 mg twice a week, no hCG , no AI. Got gyno symptoms after about 2 weeks. My baseline E2 was 4, it bounced up to 28 on my script.

So yes, you should have an AI handy and keep the E2 below 30. Some people like me are super sensative and have to keep E2 below 25.

good question, there is a point of no return, but that depends on the individual, i been on TRT for about 4.5 months and due to some issues i am thinking of discontinuing, not sure if i can get back to where i was when i started, but we'll see. if i don't recover then i may just do a light TRT like 100 mgs per week and forget my goals in terms of muscle and body fat!

you don't need a doc in your area, you can have one at any state. I have one far way from my place, haven't see him at all, and I deal all by e-mail and phone. I'm very happy with him.
