Good Testosterone and yet display symptoms of Low T?

    Hi all,

I recently got BW done to determine what I believed to be my low testosterone levels . Suprisingly, I came back at 700 ng/dl (350-1000)with free T at 16.0 (9.0-25).

Yet I display a lot of low T symptoms: fatigue, non-existent sex drive, low muscle mass, little body hair, etc. I just feel worn down physically (not depressed).

Could anyone give me any insight on what is going on? My doc is just as puzzled as I am, as from the symptoms I gave him he was sure my levels would be low.

Could this be a problem unrelated to testosterone ? Any tips or pointers would be appreciated.

Estrogen, dhea, pregnenolone, TSH, T4, T3, progesterone, cortisol, shbg, B12 and other deficiencies? Need bloodwork, what you have given us is a guessing game, but most likely high estrogen. Get all those tests as a barometer and baseline at least.

Thanks for the input - perhaps a full panel will help. I don't really see why it would be high estrogen. I have very low levels of body fat, always been a naturally skinny guy, no gyno or anything that would indicate excess estrogen. Hmmm.

Everyone is different for sure--I'd recommend getting those tests so we can help you help you... And your Dr.

I agree with the previous posters. A lot of things can cause low testosterone symptoms, so you need to dig deeper. What the other guys did not mention is a complete iron panel to rule out iron deficiency or iron overload.

Can high estrogen be an issue, if free testosterone is good (from what I can tell)? I`m really curious about this.

yep need full panel...if you SHBG is high for some reason it doesnt matter how much testosterone you have- none will be available to your body- so get your dr to test you for everything, tke a look at my bloodwork panel in an earlier post it wil give you a good starting point.

Also have your thyroid checked for hypothyroidism.

+1 on thyroid. Thyroid problems have most of the same symptoms as low T.

    Thanks everyone for the input.

One question I would have for you all is about thyroid. Since I am genetically extremely skinny, wouldn't that rule out hypothyroidism? Wouldn't I be hyperthyroidic, which would lead to increased energy? Or am I looking at this the wrong way.

My diet is normal. My stress level is normal or even low. Simply put, I just feel... physically burnt out. I yawn all day and feel exhausted even with 8-9 hours of sleep. I can get it up for a girl, but the drive to do so just isn't there. I can't focus or concentrate, and will often forget something my boss told me 2 minutes ago because I distracted myself. I've got like brain fog.

I'm only 20, which is the most frustrating part.

As stated, you need complete BW done. See the Finding a TRT Doc sticky for a list of BW. After you get it, post it with ranges. Regarding thyroid issues here's a good place to start reading:

You should probably look at prolactin also. That was one of the first things my doctor looked at with the same symptoms. Elevated prolactin can mess up sexuality. One of the most usual causes is a pituitary tumor, which can mess up other things too. If that's the case it's usually not dangerous to correct/treat.

What are your diet and sleep patterns like, my levels are identical to yours. My SHBG and E2 is midway.

Do you have any inflammation? Joints, intestine or skin. If so, this could be decreasing your overall health thus presenting you Low T levels.

Also do you use any stimulants? Geranium, caffeine, etc

I agree with all others, do a full hormone panel.. also keep in mind all other factors stated above.

Still curious about this if anyone knows. May have to start a thread about it.
