Rly need advice if I need trt
Hi 24 years old here 6 foot 200 pounds decent shape 15% body fat been experiencing a lot of symptoms of low t such as no sex drive at all ED issues with gf maybe feeling good once every couple weeks about it, lethargy, no motivation. Thought I would get a blood test done from my fam doctor results were :
TSH: 1.920 uIU/mL (.270-4.20 uIU/mL)
(T4): 7.6 ug/dL ( 4.5-12.0 ug/dL)
(FT4): 1.20 ng/dL (.83-1.62 ng/dL)
Total Testosterone : 349 ng/dL (249-836.0 ng/dL)
free test: 8.92 pg/mL (8.80-27.00 pg/mL)
Vitamin D 82 ng/mL (32-100 ng/mL)
Got another test done
TOTAL test: 313 ng/dL (250-1100 ng/dL)
FREE test: 64.1 pg/ML (46-224 pg/mL)
BIOAVAILABLE test: 134.6 ng/dL (110-575 ng/dL)
SHBG: 17 nmol/L (10-50 nmol/L)
LH: 6.0 mIU/mL (1.5-9.3 mIU/mL)
PROLACTIN: 18.2 ng/mL (ng/mL)
Any advice? Seems like im primary hypogonadalLast edited by knicks20; 08-02-2012 at 08:12 AM.

What's up brutha, welcome to the forum. Can you do us a big favor and edit that post with the reference ranges for all the values?

guessing first free range is something like 8-24 and second is 50-150. but could be way off, or it could be 2-24 on first. Anyways free test is really important and if at bottom of range could be a huge contributing factor. But like xtitan said post the ranges. Based on this you really should consider doing something about it or exploring it further. My insurance bases coverage on total test being under 300. So that might be an issue for you too. Insurance wouldn't even cover costs of blood tests for hormones because I'm too young. So things to consider, hopefully you have some income to handle this. Test shots really are cheap, HCG not too bad with the right pharm, and since TRT HCG doses are relatively samll compared to say people doing the diet thing.

I got a brain MRI done came out negative for a pit tumor, also one doctor said one of my testicles is a little smaller and that might be causing low t is that possible?

yes its possible, your LH is okay but at your age it should be a little higher, FSH can tell us more. its hard to tell with these tests alone, you need to do a full male hormone panel. see Finding a Physician thread in this section to see what panels you need.

Prolactin is high, needs to come down. That alone could be the culprit with your ED and sexual related sides. Suspect your E2 might be elevated as well. As suggested, run a complete set of labs, not a partial. At a glance, one might indeed suspect the condition to be primary.

Got another test done and here are the results
TOTAL TEST: 124 ng/dl Range: 250-1100 ng/dl
FREE TEST: 23.7 PG/ML RANGE: 46-224 ng/dl
LH: 3.4 mIU/mL Range 1.5-9.3 mIU/mL
FSH: 1.3 Range: 1.6-8.0 mIU/mL
PROLACTIN:10 ng/dl Range: 2-18 ng/dl
Estradial: 28 ng/dl
Pretty shocking to say the least this test was done at 2pm instead of the usual 9am is 10 still high for prolactin?

should always test in the AM for your natural levels. but it looks like your nuts work, but you aren't sending enough signal to them. That's low enough to ruin your day and about most every day. Mid range on prolactin but at same time, I don't know if there's a general consensus on if you should be one side of the range or the other.

Yea strange how my LH dropped but in my first test it was 6.0 mIU/mL (1.5-9.3 mIU/mL) which seemed decent enough just not enough test being produced

Hahahaha, yeah, maybe somebody is performing brain surgery on you while you're sleeping?

Id want to get another test done but at the right time this time.
Regardless, your T is on the low end, but that doesn't necessarily mean you need to be on TRT. Your first number wasnt even out of range so you're certainly hopeful given your age. The severity and duration of your symptoms, of course, are to be heavily considered as well.
