My main symptom right now is lethargy . How can my free test be so high while my shbg is high and total test is low. I had a feast the day before (10k cals) as well as had sex if that has any input.

Lab error. Your lethargy is confirmed with those lab numbers. Your cortisol, if that is AM, should be 2-2.5 times higher. Free Testosterone (4.2) should be 5-6 times higher. Your vit-D and FT4 are the only decent numbers. You've got some work to do!

results taken at 2:30pm. How am I to be sure it was a lab error?

Lethargy is complicated, and not always related to testosterone . It plays a part, but just a part.

Elevated SHBG is usually just an indicator of other things in the body. The fact that you have high free T and very high SHBG yet low total T is quite odd. I would be looking at my labs again to make sure I read it right! Other than that, I might just be confused and get rechecked.

Your thyroid changed a lot, what meds are you on?

I'm not an expert...but I'm pretty sure it is nearly impossible to have a TT of 381 and a SHBG of 67.5 with a free T anywhere close to 26.8. Besides that, why would your free T jump so drastically from 4 to 27 with no meds? That's why I believe your free T is a lab error.

Way to high. Make sure you are supplementing DHEA and D3 to help get this down. I agree with HRT that elevated SHBG is usually a sign of something amiss in the body.

    Cortisol 7.4 2.3-19.4
    Prolactin 12.2 4.0-15.2
    These are fine.

    Tsh 1.530 .450-4.5
    T4 free 1.63 .82-1.77
These are fine but to really assess Thyroid health you need more labs.

    Estrogen total 80 40-115
    Vitamin D 25-hydroxy 50 30-100
    Noted on D3.

    Testosterone total 381, 348-1197
    Low. How old are you?

    Free 26.8 High, 9.3-26.5
Given you elevated SHBG this makes no sense.

    Estradiol 24.8, 7.6-42.6
    Fine.

You are a candidate for HRT but I'd like to see more labs as noted in kel's sticky on finding a TRT Doc.

21 years old. Yes the free test is throwing me off considering shbg is almost at 70, i guess ill chalk this up to a lab error as i am getting re tested tomorrow. Will post results here.

Will be interested to see these updated numbers. Good luck. Don't work out the day before or get drunk!
