Questions about chronic prostatitis and chronic epidydimitis
Hi,
I have a question about prostatitis and epidymitis. I was diagnosed with these two conditions 15 years ago and I get flare ups every few months. I'm 38 years old. I get a dull ache in my right testicle (3 out of 10) and that's is a daily issue. It flares up to an 8 (on the pain scale) a few times a year. I'm prescribed motrin 600mg's and ciprofloxacin for a week so the flare up subsides. I've had ultrasounds done and was found to have a spermatocele. It's a small one and the docs don't want to remove it or drain it because it opens me up for infection. Also, my doc said that when you operate someone to relieve pain, you usually increase pain so he said to leave it alone. I've had several urinalysis' done and they never find the infection or bacteria. Digital rectal exam finds tenderness on the right side of my prostate, the same side as the testicle pain. Because of this, the diagnosis is chronic prostatitis and chronic epididymitis.
Last year I was diagnosed with low testosterone . I was diagnosed as secondary since I have low-normal to nomal LH and FSH levels. My estradiol is good, my thyroid is good, my cholsterol is good and so are all my other numbers, except for testosterone , which is in the 270's on one test and in the 290's on another (both taken in the morning). I don't have my labs with me, I'm at work.
Could this be the cause of my low t? I was thinking it might not be because of my testicles were damaged, wouldn't my LH and FSH be high? In the past, I've been on painkillers but never for more than 2 or 3 weeks. I've never been on for a long period of time. I've been on prednisone but never for more than a couple of weeks and that was 10 years ago (same as with the painkillers). I'm trying to figure out why before I jump on TRT.

I'm 6 feet tall and 210lbs. I work out regularly and with heavy compund lifts. I eat well and don't drink. I sleep well every night. I really don't know how low t could have happened. I also have 2 bulged discs in my back but that hasn't stopped my performance (I deadlift 500lbs for reps and squat 325lbs for reps). I haven't touched a painkiller for my back since 2002. Besides motrin, I don't touch a painkillers.

At those numbers, it's quite clear you have low T. The first move anyone would advise is to get thorough diagnostic testing done to find the cause. Putting all the results together with what you already know will help you know how to proceed.
From my own experience I've had trouble with both of these when I either had too much sex or too little.
As a humorous side note, the only time I had a dull ache in my testicles was when I had "blue balls" - for lack of a better term.

Hi 2Sox, thanks for chiming in. I've read some of your threads and wanted to thank you because I went to see Dr. G. He tested me and while waiting for the results, I found some of your threads and decided against going to him. I found a doctor in Manhattan that lets me self inject and prescribed an AI and HCG . He started me off with a 10ML vial of test C and 10,000 iu's of HCG. He said to inject once a week but after reading around here, I'm going to inject 250iu's of hcg on Mon and Wed and 50mg's of test on Tues and Thurs. I dind't start yet because I'm still reading a researcging to make sure that this is the lifetime journey that I want to committ to.
I have more tests at home, I'll post them when I get there. Like I said, they checked thyroid, liver, cholesterol, cbc, test total and free, estrogen, lh/fsh/etc. Everything was great except for the free and total test. I'm still trying to absorb all of this because, quite frankly, it's very disapointing to hear.
I'm still wondering if the prostate and epididymitis could be the problem. From what I've read so far, prostatitis can cause erection problems since the nerves originate from the prostate but I see no connection to low testosterone . It makes it even harder for me to connect since my LH/FSH is good. I've seen about 8 urologists over the past 15 years and they all say the same thing. Leave it alone and manage the pain. Having invasive surgery to fix pain is not recommended.
Sex is fine and having too much or too little doesn't really affect the pain. Sititng for long periods or tight underwear does. I make sure htat I don't sit for long periods and I wear boxers now.

Dr Joseph Olivieri. (212) 406-0134. 139 Fulton St, Suite 707, NYC. He takes my insurance, BC/BS and everything is covered. He gives me scripts for everything and runs all the tests, checked my heart, full labs, etc. I dind't have to fight with him either. I asked for HCG and he asked why. I said I don't want my nuts to shrink. He filled out the pre-auth form and handed me a script. I see him every other month and get a new script for test, HCG and arimidex .

Spoke to the doc and he said that I'm secondary. He said that he doesn't know why since there is no drug abuse or diseases in my past. I've never been knocked out or in a car accident. Don't drink or smoke and my labs are good. He thinks it's just me getting older or just a bad draw in the genetic lottery. I do have 2 bulged discs in my back but he said that 70% of the population has bulged discs and since I can still workout and lift weights (squat 350 and dealift 500), I don't really have back problems. When I mentioned the prostate and epididymus, he said that it's just chronic pain and there is no bacteria and when he checked my history, there never has been. Also, sicne he said I'm secondary, those wouldn't factor in anyway. If I were primary, then that would be a different story.
I asked about a tumor and he asked if I get headaches or if I have a narrowed field of vision. I don't so he doubts that I have a tumor.

I feel ya. I had a scope up there once. No problem with the procedure but the doctor neglected to tell me to drink plenty of fluids for a week to dilute the urine. As a result, every time I urinated it felt like I was pissing fire. This went on for a week or so!
I hope you feel okay. No need to tell you to keep an eye on things.
