Blood Work Results after 6wks on TRT

I had my 6 week mark labs performed on Friday and recieved the results this morning. I am currently on 200mg Test Cyp weekly. I split the IM injections into 2X100mg Mon/Thurs. HCG is taken Mon-Wed-Friday 250iu. I have not been prescribed an AI but seeing that my sensitive Estradiol came back at 76 I am going to be speaking to the Dr about starting on an AI.

I have been feeling pretty good until about the 5 week mark when I started to feel a bit off again. Started feeling a bit lethargic and libido and erection quality has dropped off a bit.

    Stats:
    46 Male
    Starting weight 214
    Weight 6 week mark 232
    BF 20%

I have high lighted the flagged results. Besides the high E what other issues should I address with my Dr? Thanks in advance for the feedback.

Probably when your E2 stated to elevated when you started your protocol. Read the sticky on Estradiol and Management. Make sure you really understand it when you speak to your Doctor in the event you get push back. May want to print it out and have it with you if you think it there might be some resistance

The weight increase could be as a result of your elevated E2. Do feel like you are retaining any water?

I have high lighted the flagged results. Besides the high E what other issues should I address with my Dr? Thanks in advance for the feedback.

This is at the top of the reference range and if you're Doc is astute he/she will see this and know that you need to give blood to get this down. Elevated Hematocrit is not uncommon when a man is on a TRT protocol so start giving blood and regularly as well.

Nice! Right at the top of the reference range. You had your draw on Friday, did you inject on the Thursday before? You may have been better off getting your draw on Thursday before your injection Nevertheless, twice weekly injections provide for pretty consistent serum levels so you probably live at the top of the reference range anyway.

    Free Testosterone(Direct) 34.4 High pg/mL 6.8-21.5

Elevated but probably more related to your injection the Thursday. It's up there because your SHBG is in a very nice range so your not binding up excessively. 200 mg of Test a week is at the top of the TRT dosage range so you may want to discuss lowering your dosage when you talk to your Doc. Don't make a huge reduction in dosage amounts, work down slowly and allow time for your pathways to adjust accordingly. The HCG is probably adding in natural production as well and another reason for the elevated level. Don't fool with the HCG dosage, just titrate down on your Testosterone dosage till levels come down into the normal healthy ranges.

    Hemoglobin A1c
    Hemoglobin A1c 5.1 % 4.8-5.6

    TSH 1.250 uIU/mL 0.450-4.500
    Thyroxine (T4) Free, Direct, ST4,Free(Direct) 1.06 ng/dL 0.82-1.77
Need to see more Thyroid labs to make any assessment but these don't send up any red flags.

    LH 0.1 Low mIU/mL 1.7-8.6
    FSH 0.2 Low mIU/mL 1.5-12.4

These are tanked because you are HPTA suppressed and it's normal. Read the HCG sticky.

    Prostate-Specific Ag, Serum
    Prostate Specific Ag, Serum 1.7 ng/mL 0.0-4.0
    Fine.

    DHEA-Sulfate 209.6 ug/dL 44.3-331.0
    Fine. Are you supplementing DHEA?

    IGF-1
    Insulin-Like Growth Factor I 268 High ng/mL 64-210
Lucky Boy! No HGH for you pal!!!

    C-Reactive Protein, Cardiac 1.74 mg/L 0.00-3.00
    Estradiol, Sensitive 76 High pg/mL 3-70
Too high, lower your Testosterone dosage as noted and talk about adding in a low dose AI to get this down into the 20's. This will also help increase your Testosterone serum levels meaning that you can reduce your Testosterone dosage levels further based on testing of course.

    GGT 40 IU/L 0-65
    Magnesium, Serum 1.9 mg/dL 1.6-2.6
    Insulin 5.9 uIU/mL 2.6-24.9
    Ferritin, Serum 23 Low ng/mL 30-400

Sex Horm Binding Glob, Serum 24.3 nmol/L 16.5-55.9
Perfect, right where you want it!!!

Thank you for the feedback G! I have included additional info in bold.

"Probably when your E2 stated to elevated when you started your protocol. Read the sticky on Estradiol and Management. Make sure you really understand it when you speak to your Doctor in the event you get push back. May want to print it out and have it with you if you think it there might be some resistance" I am onboard with lowtestosterone.com so I am hoping that I will not get any pushback from the Dr.

"The weight increase could be as a result of your elevated E2. Do feel like you are retaining any water?" I definitely feel like I am retaining water. I can see the white thumbprints on my skin, especially on the upper chest. Also my face is a bit puffy.

"You had your draw on Friday, did you inject on the Thursday before?" Actually I purposely did not inject on Thursday due to the draw on Friday morning. I injected after the draw on Friday afternoon.

Are you supplementing DHEA? I am currently taking 50 mg of DHEA daily as well as a handfull of other supplements. Thank you for your supplement thread!

Should I be concerned about the Iron/Ferritin results?

    Thanks again.

Ferritin levels determine how much iron you have in your system. It could be low due to many reasons. One is becoming anemic. Another could simply be due to hemorrhoids (serious.)

I've recently dealt with low iron myself and it can make you very lethargic and tired and feel like you're not recovering from your work-outs. I've begun supplementing with elemental iron and it's made a big difference for me. Before you supplement you should talk to your doc and pull a full iron panel.

GD covered all the rest very thoroughly.

    >The Doc's at LowT know what they are doing; they will put you on a low dose AI.

    >Once you get your E2 down you will lose the water weight.

"You had your draw on Friday, did you inject on the Thursday before?" Actually I purposely did not inject on Thursday due to the draw on Friday morning. I injected after the draw on Friday afternoon.

    >In this case you need to talk about lowering your dosage. Clearly your current dosage is too much. Consider that 100 mg per week, even less, are the normal starting dosages by some of the leading TRT Physicians.

Are you supplementing DHEA? I am currently taking 50 mg of DHEA daily as well as a handfull of other supplements. Thank you for your supplement thread!

    >Welcome.

This is exactly what I have been experiencing. Maybe a combination of low iron along with high E2? I will definitely address the issue with the Dr.

Thanks again guys. I appreciate all of the advice/feedback.

Your dose is too high if your trough still has you over the range, if you lower it your estrogen will follow. You prob will still need an ai, but only way to know for sure is adjust dosage, check bloods and see how you feel. Definitely talk to your doc about the iron situation. Good luck brotha, by the way 18 pounds gained in 6 weeks!???
