You would have to be fairly retarded to die from insulin use.

well death is obviously a bad one
but what else? perhaps becoming insulin dependent?
other conditions?
personally I think that I wont be using insulin for more than 3 cycles in my life....
its very affective and has helped me recover my strength quite a bit
I stopped lifting for 3 years and when I stopped I weighed 230lbs with around 10%bf
My strength is 90% back since I started using insulin as an anabolic
I've been using it without steroids (alone) and has noticed my strength come almost all the way back

Even though athletes have ended up with diabetes in the past, there is no actual proof that insulin use can lead to dependancy. Most bodybuilders who use insulin also use HGH, IGF insulin and AAS which all effect your blood glucose so perhaps they're all to blame, not just insulin.

I run insulin all the time, usually 4-6 weeks on, 4-6 weeks off........i find it very beneficial to use during PCT and also when not on a cycle....
