Hypogonadism ?

Can someone tell me the cases which bring permanent hypogonadism ?

I know I've read at least one thread by a fellow who got it from a cycle (he recommended HCG which is why I use it religiously DURING a cycle) and I have a buddy who says he has one nad that just won't come back to size from using gear. Some guys don't seem to think it's a big deal, but I'm not one of them.
