    Dr. Ignoring E Levels?

When my Endo first analyzed my BW, it was this (43 age):

    Test Bio: 23 ng/dL
    Test Total: 24 ng/dL
    Estrogen: 111 pg/mL

Following 3 months of TRT (1st patch and then cream), the numbers were:

    Test Bio: 192 ng/dL
    Test Total: 269 ng/dL
    Estrogen: Not tested (??)

In the interim, he has changed me from cream to TestE 100/week.

Bloodwork ordered for Feb 1st but once again no Estrogen test. Can E levels be inferred from one of the other tests?

No, it must be tested specifically.

From my experience, most Endo's don't know how or aren't willing to directly address Estrogen issues in men. Their typical solution is to just lower the testosterone dose. Most do not support administering an AI 'as there is not enough evidence to support the benefit to risk ratio in men for purposes or reducing Estrogen.'

I'd press him/her to test your E, especially if you are having too high E related issues, but nonetheless, to see where it is at.

I guess the question is whether my E was high (111 pg/mL) to begin with. The lab results say that an adult male should be below 200pg/mL. Linearally, 111 would be middle range. Not sure how to interpret this.

At that test I believe you are correct based on pg/ML. I don't normally work with pg and had to look it up (usually see ng/DL). It would be nice to see your current level of E as you say. Do you have any symptoms of high E? Puffy or irritated nipples? Bloating, abnormal fat gain, especially around butt and hips? Abnormal mood swings? Libido issues? Confusion or memory issues?

Libido has been in the tank and the TRT has not improved this to date. Its funny that you ask about the Gyno. I have been double dipping on my therapy...adding one tube of Testim gel per day on top of my 100mg/week of TestE shots. Ever since I began doing this, I swear my nipples look a little bigger.

good advice above...i might add that e2(estradiol) is a better test for trt when looking for your sweet spot (test/estro ratio)..

so youre saying you havent felt better since starting? is libido the only thing ur complaining about...how about mood, energy, strength? do u workout/exercise?

I feel like a million bucks minus libido. Actually, I am much, much better than when I was cycling with AS, which is what caused the low test. I am going to experiment with some Tadalafil for the ED, but heck, I don't wanna be taking so many drugs you know?

My endo has never tested me for estradiol levels in a year worth of visits. I'll ask next visit what his thoughts might be. I suppose if your free testosterone levels are good and no other issues are present, you might be able to infer that estradiol isn't a problem. The libido problem suggests to me that more investigation needs to be done by your doctor. The tadalafil can be very helpful as long as you have an interest in sex.

    sirp
