19 with life-long erectile dysfunction and low libido.

Hi guys! I'm 19 years old, currently living on Long Island, am 220lbs, 5 feet 9 inches tall, have suffered in silence with weak erections and a low libido for my entire life. Essentially, I feel as if I never fully went through puberty. I finally went to see a urologist in the beginning of this past summer. Doctor just ordered total testosterone and that was it. I scored a 268 with a range of 250-1100. He diagnosed me with "low-normal testosterone" and said I could try either gel or injections if I wanted. He put me on 300mg injections every other week. I noticed very marginal improvements in my sexual life. I would say about a 10% improvement. In other areas, mental, and physical, I would say a 75% improvement. I was going to the gym, more energetic, happier, gained some muscle, more confident, etc. Well, at our 6 week checkup, I voiced my concerns of my future fertility. At this point, doctor basically fessed up and said he wants me off the shots and referred me to a different doctor. His reasoning was that we should have done more testing and it is very rare for someone my age to have low testosterone . So two months go by. I have not been on anything and get a slew of labs done by a local family doctor. Couple of things I want to say before I post these labs: I used to be a hardcore long-distance runner from the ages of 12-16. I was very skinny and I sense that may have contributed to my ****ed up HPTA. I realize I am not in the best of health right now. I am a bit overweight but I eat very healthy. I take 10-20 grams of fish oil, and a multi-vitamin, it is just very hard to be active with such low energy levels. Last year, I bought a couple of bottles of Toremifene from a reputable research chemical website. I ran 60mg everyday for 3 months. I noticed absolutely no improvements in anything. I do not have labs to back this up though.

Here are most recent labs from two weeks ago(off of everything):

    FSH: 1.0(1-11)
    LH: 1.2(1-8)
    Cholesterol: 208(under 170) HIGH
    HDL: 41(over 40)
    LDL: 150(under 130) HIGH
    Alkaline Phosphatase: 54(60-270) LOW
    Testosterone: 119(160-726) LOW
Free Testo w/ Total and SHGB: 4.19(2.03-13.48)
    SHGB: 5.7(10-57) LOW
    %Free Testosterone: 3.5(1.50-3.2) HIGH
    Estradiol: 41: (0-56)
    Progesterone: 0.4(0.0-0.9)
    Cortisol: 14.9(AM: 5-25)(PM: 5-14) Blood drawn at 12:30PM
    T3 Total: 1.53(0.80-2.00)
    T4: 7.1(4.5-11.7)
    TSH-Ultrasensitive: 4.060(0.270-4.200)
    Free T3: 4.95(2.0-4.4) HIGH
    Vitamin D: 33(30-100)
    DHEA Sulfate: 389(108-441)
    Anti-ThyroGlobulin AB: under 20(0-40)
    Estriol Serum: 0.02(00.00-0.16)
    Prolactin: 7.9(2.5-17)

Areas of concern: testosterone is extremely low for my age. It has decreased by over 100 points because my urologist took me off the shots cold turkey with no PCT. I am feeling very, very shitty currently. TSH is borderline high which suggests hypothyroidism but free T-3 is over the range high which suggests hyperthyroidism. So which is it? These two values are completely contradictory which suggests I may have reverse T-3 dominance. Based on my research, this will be a tough sell to convince any doctor to test for, let alone treat. Apparently, this condition is not recognized in the medical community. I have an MRI of my pituitary scheduled for tomorrow. I have a feeling it will come back normal, but I will wait and see. I have a couple of appointments coming up in the next few weeks: one an endocrinologist from the "find an endocrinologist" at hormone.org and another family physician from the life-extension directory. Both were found using the "finding a trt physician" sticky. SHGB is also extremely low and no doctor has been able to tell me why. I know hypothyroidism=low SHGB but my labs suggest I do not have this, so I don't know.

Other labs from memory: Blood test taken one week after 300mg shot of testosterone: Free test was like 2x the highest range, total testosterone was around 700, highest being 1100. SHGB was also very low, around 5, the same it is when I am not on the shots.

    Hi Endo.

Sorry for your troubles! So, the above BW is after being off the T for two months without any pct. Makes me curious if pct would have done anything at all for you. Your LH/FSH are basically nil, therefore your T is as well and your shbg has nothing to bind. Whether the LH/FSH is due to the recent cessation of the T and no pct or a pathology remains to be seen.

The MRI will be a big plus here letting you know if it's tumor related or not. If it is, at least you will then know a causative factor. I have a pituitary tumor and you'll be fine if that's the case.

I'm not going into thyroid issues as your obviously on top of that other than to say a more modern range is 03 - 3.0 for TSH, not the one your lab provided. A great resource if you're not familiar is www.stopthethyroidmadness.com. I'm not seeing your T3 over the range high? Knowing your rT3 level here would help you here if it is rT3 dominance.

Do you take any supplements like zinc, selenium, D, iodine, kelp, etc?

Welcome to the forum! It's late but guys will chime in with their opinions. A couple of the guys here are very good with thyroid issues and may be able to lend a hand.

I would suspect reverse T-3 dominance as well. Sometimes TSH, FT4 and FT3 can be fine till you get to RT3 and it tells a completely different story!

    Get Thyroid antibodies as well.

You're HPTA suppressed and why LH and FSH are so low and Testosterone serum levels in the tank.

I agree with kel on the MRI as well.

You're too young and the Doc just putting you on Testosterone was a serious medical injustice...you're worse now then before you saw him.

Find a Doc who understand hormones in men and don't think an Endo is going to get it...most don't...so do you search carefully and ask a lot of questions.
