Elevated liver enzymes on TRT, any ideas?
Hi guys I'm currently on trt and got my latest blood work done and found my liver enzymes are increased I was hoping someone here could shed some light on what may be causing it in my regiment.
Bloods:
Comp. Metabolic Panel (14)
Glucose, Serum 79 mg/dL 65-99
BUN 32 High mg/dL 6-20
Creatinine, Serum 1.14 mg/dL 0.76-1.27

eGFR If Africn Am 99 mL/min/1.73 >59
BUN/Creatinine Ratio 28 High 8-19
Sodium, Serum 140 mmol/L 134-144
Potassium, Serum 4.1 mmol/L 3.5-5.2
Chloride, Serum 100 mmol/L 97-108
Carbon Dioxide, Total 24 mmol/L 18-29
Calcium, Serum 9.4 mg/dL 8.7-10.2

Lipid Panel w/ Chol/HDL Ratio
Cholesterol, Total 132 mg/dL 100-199 01
Triglycerides 86 mg/dL 0-149 01
HDL Cholesterol 32 Low mg/dL >39 01
Test, serum >1500 high 348-1197
Free test (direct) >51.0 high 8.7-25.1
Estradiol 224 high 3-70
Current protocol:
Test cyp: 200mg/wk
HCG 500x2 /wk
DHEA: 50mg / daily
vitamin d3: 5000 iu / daily
exemestane: 12.5mg / daily
B12 inject 1ml/ wk
Other supps taken:
BCAAS
10 g creatine/ daily
protein
multivitamin
pregnolone 50mg / daily
cycle support 2 scoops/ daily
I was on niacin 1500mg daily but dropped it to help with my slighty low HDL. Thought this was the culprit however dropping it did not make a huge difference.
I'm going to stop taking such a large amount of creatine and increase my stane dosage obviously my e2 is high.
I'm not taking any oral AAS, only what i have listed above. I also do not drink any alcohol. I do have a very high protein diet. Protein macros are at 350 a day for protein.
Any help would be greatly appreciated!

One thing you defiantly need to drop you test down, your levels are cycle levels. Trt is replacement, most drs try to get it between 700-1000. Your free t is also really high, I would drop down to 100 mg, 50 e3d. Do bw in a couple weeks and see where that puts you. Doing 12.5 of that particular AI should have your estrogen lower. Are you on dr prescribed trt? If you get these in check I'm thinking your liver values should come down. Remember every one is different, some guys need only .75 mg ew and some guys need 250 mg ew. You don't need 200.

Agree with PPWC. Drop your dosage back on both Test and HCG . I don't know if I'd cut it in half but I'd at least drop it to no more than 150mg's per week split into two injections as stated. These two changes in and of itself will help greatly with your E2. There's no need for 500iu's of HCG in one shot. I'd drop it back to 250iu's twice per week. Simply no need to slam them that hard.
Add in NAC at maybe 2400mg's per day to help with your liver values. I use NOW brand from Amazon. NAC is a very healthy thing to take on a daily basis. It will even help with cholesterol. Remember, simply lifting will elevate liver values for up to 10 days....
No need to eliminate Niacin, just take the correct one. Read this on Slo Niacin:

Cut your protein intake and test doses in half.

Good stuff guys.
I'm going to drop my test dose to 100-125mg 2x week (i skipped today's inject), Hcg 250iu 2x week, stane 12.5mg 2x a day for a week then to 12.5mg ed or EOD by feel, I'll increase my NAC to 2400mg, I'm also taking Liv-52 hopefully this will help.
I'll wait to start the niacin again i may have blood work for an upcoming job and need my liver values to be somewhat in range. I know they're drug testing not sure if they check hormones or not. I just dont want to risk it. Great article though I'm a big fan of niacin.
Is there anything else I can do? Do you think my high protein intake is also effecting it as someone above mentioned ? I've always had high protein intake previously and never had an issue with the liver values so i dont think this is it.
Is there anything else or do you think this should solve my issue?

The high protein intake could also explain your high BUN.
There's no way you need 350g of protein.

With the NAC that's enough you don't need the liv-52.

Just so you don't get confused. .
Lower the dosage to 150/wk.. which is 2 injections of 75mg.
You said 100-125 2x a week which would not do what you're after

My pre trt enzymes were off the chart high. I had a cold and was taking day quil.
My 6 months were perfect.
The liver heals itself, on your next course of blood work if its still high, then perhaps you should have it analyzed by a Dr

I spoke with my HRT Dr yesterday and he does not think the elevated liver enzymes are Test Cyp related. He agrees that it may just be because I lifted super heavy the day before.
I also had another theory. I changed up my diet recently and have been eating a shit ton of that puffed wheat cereal just because the macros are desirable. When i say shit ton i mean like a 6oz bag every 2 days worth. Well i also noticed that I had a lot of stomach discomfort and disgusting gas all day. I started looking into elevated liver enzymes and wheat intolerance and at the more extreme level celiac disease. In my research I read that if you do have a wheat allergy it can raise liver enzymes significantly.
Have any of you heard of this?
I started thinking outside the box because my liver values weren't even that high after a high test, tren , var cycle. The only two things I changed were immediate release niacin 1500mg daily and then the wheat cereal. It's not the niacin because I got a second set of bloods with no niacin for 4 weeks, liver values still high.
For the passed 3 days I've been trying a gluten free diet and so far no stomach discomfort and flatulence has decreased significantly, I just cant believe that wheat could really do that.

I had mine come back and they are similar I dont think you should worry doctors see people with the numbers in the 1000's on ast alt, up to 1500 everyday people who don't exercise smoke etc: and yours like mine is slightly raised I dont drink and have a careful diet with fat intake watched; just keep testing to keep an eye on it with bloods the liver is very resilient

Also take into consideration , age may play a factor as well.

Is OP taking any pain meds like Tylenol or Advil? If so, this would definitely result in elevated liver values. I'd guess the culprit is more likely the supplements over the 200 mgs/week of Test Cyp...

Op, u could be reading way too much into this, keep in perspective, it is only one test.
On that note, I no for "almost" a fact my creatine usage screwed my kidney functions tests good, all came back to normal when I discontinued.
Make the recommended changes and get retested. Curious, when was your blood draw in relation to your last pin?
