i saved a pregnant chick from getting beat up by her hick ass boyfriend one time
middle of philly street. ppl just standing by watching this guy smack her around and shake her as she cried.
so i stepped up and wrapped my arms around his arms and locked mine behind his head. tripped him and sat on top of him as he was face down in the street till the cops came

i broke up a fight one time cuz this big dude was beating up on this little dude
then the big dude hit me and i ended up kickin his ass and after it was over he came back with a bunch of friends to jump me. but while they were still in there new durango ( we were 17 so it was their dads)
i walked over to their car with a wrench and started shattering windows. the driver started crying "stop its my dads. please stop!!!!"

I have learned that there's something to be said for letting others settle their differences without my interfering. I have scars and fake teeth to prove it. I'll still consider getting involved for a friend, or if the playing field is unfairly tilted, but last time I saw two dudes start to fight by the side of the road I stopped just to watch.

best you can do is ibuprofen without seeing a doc. and sleeping like a bat.
you might want to get some insurance in case it doesn't get better in the next week or two.
Next thing you can do is get a script for prendisone, much stronger than NSAIDS.
Then MRI
Then come the cortisone facet injections. Needle right to the spine. Knocks out any inflamation in there.
Still pain, surgery.

what they do is go in through your belly and they do what's called a retro-peritenal aproach. So, they go around your intestines and the tissue holding them in place. Get to the spine (all this is done by a general or vascular surgeon), move the great blood vessels (vena cava and doral aorta) off the spine. Take out the disc. Most commonly L 4-5 or L-5-S-1, often both, once in awhile L 3-4, very rarely L 2-3.
Then if you can imagine a cross section of your femur, it would be a ring. They take a donated femur from cadaver bone, that's machined to the right height and put it where the disc once was. It takes a special machine to get enough leverage to shoe horn it in there. In this case we were going to use a carbon fiber cage filled with BMP, but does the same thing...It's a graft.

so all that is done while you're laying on your back
It's a special table, that they can build another table on top of you and then rotate it to flip you over on your belly.
They put you in a doggie style position, then cut you down the middle and get to your spine. Then put pedicle screws in and high speed drill the facet joints and stuff them with bone graft from your own body or BMP. Also they often fuse transverse process to transverse process by drilling them first and laying down graft.

so you go from laying on your back to doggie style to laying on your back when they throw you back in bed to the recovery room. Hence the name 360 fusion.
