Help with Methylcobalamin (B12) injections
Hey Guys, doc just gave me a vial of methylcobalamin 1mg/ml 30 ml.
What would you consider to be a good dosing protocol? I was prescribed it originally to use when I reconstitute my HCG .

My doc changed pharmacies now that APS will not ship out of state.(I don't think any pharmacy can send out of state pretty soon) So the only thing that my blue cross blue shield won't cover is HCG . This new place has b12 that's mixed in with the hcg. I just ordered it today so I don't know much about it yet but I think it's the same thing as what u have. It's from a CC pharm, cost $60 for a month supply, I was told if they use the b12 with it then its cheaper? Don't know if that's valid or not just heard it from the nurse on the phone.

there are a lot of ways people do b12 injects. I'm not sure if there is any best way. the end goal is just to make sure your b12 levels stay high.
so, if you take too much your body will store some and pee out the rest. if you don't take enough, your b12 levels will not rise or possibly continue to fall. there's no secret really. you just need to get blood work to make sure you stay in the target range.
I would ask the doc what they want, but chances are it's probably just 1 shot a week of 1 mL (1mg total) or every 2 weeks. they often prescribe a higher dose to start just to load up your low levels.

Agree with HRT. Can't take too much. So err on the over side.
Side note can't believe how expensive this stuff is in the U.S. If you ever get to Thailand you can get a years supply for $9 !

The manufacturers have really dropped off production. As far as I know, there's only 1 maker now that's making it for the major pharmacies (CVS, Walgreens, Walmart). It's sad too because an old timer pharmacist talked to me about it saying he believes they aren't making it just because there is not enough profit in it.
What I do know is that I am only able to get the 1mL vials now and the cost has skyrocketed (relatively; b12 is still quite a cheap item) through insurance.

Guys I'm not trying to brag, but instead am just sickened by the state of prices of pharmaceuticals in America. This notion of a shortage of injectable B12 is IMO a farce. I have never had a problem with procurement and the prices have always been dirt cheap.
By the way I was wrong when I said $9 for a years supply. It's actually $9 for 50 Ampoules. So what's that 2 years supply? To say there's no money in it selling at I'm guessing $30 a dose in the U.S. (I'm buying it for .18 a dose) is saying the pharmacies are expecting huge returns to make it worth their while.

People are commonly deficient in B12 so it's a neat way to get some.
The powder HCG needed some kind of liquid to dissolve in.....

Are you sure about subQ? I just asked my hrt doc last week. He said testosterone ok for subQ, but B12 has to be IM. Would sure love it if he was wrong. Can you please provide some backup to this claim?

Strong claim and Docs can be wrong, but since my Doc is one of the top hrt docs in the U.S. if not the world I'll stick with his opinion until I see strong proof otherwise.

Not a strong claim. Very common. Here's is my prescription vial, as HRTstudent stated, it clearly states for IM or SC use.
I think your doc probably just prefers IM. but if he is saying no subq whatsoever, he is very far from 'top doc'. This is very entry level knowledge.~ PLEASE DO NOT ASK FOR SOURCE CHECKS ~

He never said "cannot". It may have to with which he believes is more effective.
Here's the results of my last 3 blood draws the first was before I started, the second was injecting 1000mcg every 10th day, the last was 500mcg every 10th day. IM of course.
Vitamin B 12 (800 - 2000) "390", "1230", "808"
Would you be willing to share your labs? If your numbers are good doing subQ, I"LL SWITCH STARTING TODAY !
Next time I talk with him I'll ask why he wants IM. Don't really want to get into defending him, but look him up: Dr Rothenberg. He trained alot of the top HRT docs in America and is very well respected in the field of HRT.Last edited by fm2002; 06-20-2013 at 03:54 PM.

I'm not really concerned with your doctors credibility, I just wanted to give you info that you're more informed when speaking to him. I think you guys just had a misunderstanding is all.
Your test was for B12 looks like unsaturated serum binding capacity (based on those ranges), not B12 serum levels. Serum levels ranges are generally 200 to 1000 pg/ml.
My levels aren't really something you want to compare to. My results show >1100. I use B12 more than most desire to. Sometimes up to 1000mcg daily.
I inject subQ if all Im injecting is B12. If I mix it with testosterone , I go IM. Because I don't like injecting more than 1/2 CC subq. So I alternate depending on what i'm injecting that day.Last edited by austinite; 06-20-2013 at 04:52 PM.

I have used the exact b12 in first pic above. After injecting sub q I had bruising and pain for days no matter how slow I injected. Spoke with the doctor about it thinking maybe I was allergic. He said a lot of people were having same problem and had them start injecting it im.

Interesting. Never heard of that. I seriously doubt that "a lot of people are having the same problem" considering the diagnoses was an allergic reaction.

How administered
Methylcobalamin injections are typically given once per week for 10 weeks to restore deficient levels. Methylcobalamin can be administered by subceutaneous and intramusclular injection.
Common Protocol: Inject 1-2mL of Methylcobalamin Intramuscular (IM) once per week for 10 weeks, then once every 30 days to maintain levels.
*Note : larger amounts of methylcobalamin are necessary to correct neurological defects and protect against aging
Injection vs Oral
Absorption of methylcobalamin is optimal with an injection compared to oral administration, and therefore less frequent injections are needed once blood levels of B12 reach an acceptable level.
Research study
Oral supplements of vitamin B12 appear to correct vitamin B12 deficiencies as well as B12 injections. However, in order to correct a deficiency, oral doses need to contain more than 200 times the recommended daily allowance (RDA) of vitamin B12. Study author Dr. Lisette C. P. G. M. de Groot of Wageningen University in the Netherlands explained that most people develop vitamin B12 deficiencies as a result of "malabsorption," in which their bodies become unable to extract vitamin B12 from food. The deficiency typically strikes older people, she added, and takes years to develop. In some instances, people who avoid animal products -- such as vegans and followers of a macrobiotic diet -- can also develop a deficiency in vitamin B12 as a result of not eating enough B12-rich foods. A vitamin B12 deficiency is typically treated by monthly, often painful, shots. To investigate whether an oral dose of vitamin B12 works, as well, they tested various daily doses of oral vitamin B12 supplements in 120 people aged 70 and older. They found that daily oral doses of 647 to 1032 micrograms of vitamin B12 appeared to correct the deficiency. The current RDA for vitamin B12 is 3 micrograms per day. Side Effects:
Irritation at the injection site and/or skin rash have been reported by some patients.
