If you are not constantly improving in weight lifted or reps completed, can you continue to grow, even if the workout remains difficult?

Yes you can , But maybe try to shock the muscle into new growth with supersets . Perhaps your body has got stale and used too the same workout you do, As muscle adapts and gets used to the same thing very quickly . Its not all about max lifts its about training smart and hitting every angle you can.
